Community Placements

Beautiful Life partners with several homes, landlords, and community partners throughout the Southern Nevada community that offer shared living for adults. When an adult is ready to leave the incubator, the case manager provides comprehensive transition planning to ensure a successful move into either independent community housing or a shared housing arrangement. This includes assisting with housing applications, coordinating move-in logistics, arranging furniture and household necessities, and ensuring the resident is connected to ongoing medical care, behavioral health services, transportation, meal programs, and other community resources. 

For residents entering shared housing, the case manager carefully considers compatibility factors such as lifestyle, personality, mobility, and support needs to create safe and positive living arrangements. Following placement, the case manager continues to provide follow-up services, home visits, and regular wellness checks to monitor housing stability, address emerging concerns, and help the resident remain successfully housed while maintaining independence and dignity.
